Curious about losing a twin

A few months ago, my significant other had a "clot" fall into the toilet and she was concerned about the size. She called a doctor's office and when she brought the sample to a gynecologist per their request, based on the inspection, they informed her it was most likely a tiny fetus. She would have been about 4 weeks at that point. Upon giving her an exam to check and make sure there were no other issues, they discovered that she was still pregnant, and an ultrasound confirmed a baby's heartbeat.

We took a while to decide what we're doing, and now upon revealing to family and friends, some concerns have been raised as to the validity of the notion that her cervix would open up and allow for a tiny partial fetus to pass through, while still keeping the other baby inside and ok. Again, at most based on the most recent opportunity for conception prior to the "clot" issue, she would have been at about 4 weeks pregnant.

I don't know where to begin even looking this sort of thing up, for my own sake, so if anyone can either point me in the right direction or post information relative to how the body works if that was the scenario, please do. Thank you.
