Throbbing Left Ear Pain - No Infection

I have been having this painful throbbing ear pain on and off for about three years. I have mild loss of hearing, mild dizziness and tinnitus. I had it checked by an ENT a few years ago but they didn't know what was causing it. It is now almost constant. Anyone have any idea what it might be. It is not TMJ, or dental related.
